To obtain a Senior Citizen ID card you will make an appointment at your local DMV office and provide the following documents:
- Form DL 44 completed*
- Birth Certificate of other acceptable document
- Provide your Social Security Number
- Your Full Legal Name

The Senior Citizen ID Card is free if you are 62 or older.
